Coming home from the hospital after surgery or illness
VNA of Englewood’s registered nurses evaluate each patient in collaboration with their physician to develop a plan of care customized to their needs and to help restore independence and quality of life. Patient care plans could include wound care, medication management, ostomy care and rehabilitation therapy from a multidisciplinary team consisting of registered nurses, rehabilitation therapists and certified home health aides. Registered nurses also arrange for individuals to receive medical supplies and equipment that can assist in recovery and quality of life.
Having joint replacement surgery
If you are scheduling joint replacement surgery for your knee, hip, or shoulder, talk to your surgeon about getting home rehabilitation therapy. Following your surgery, our team of physical and occupational therapists provide therapy service at home under the direction of your orthopedic surgeon. Your therapist also conducts an evaluation of your living environment to suggest modifications or adjustments to help you avoid injury or falls, which is an added benefit of receiving rehabilitation at home.
Recovering from a stroke
The process of recovering from a stroke usually includes recovery, rehabilitation, and the return to community living. Because stroke survivors often have complex rehabilitation needs, a customized home health plan of care can assist recovery in a familiar environment and help educate caregivers who often play an important role in recovery.

Individuals who take four or more prescription and/or over-the counter medications each day, may benefit from our medication management services. Participants collaborate with a registered nurse to review their medication regimen to help avoid medication errors and dangerous drug interactions and to ensure proper administration.

Whether you need round-the-clock care or assistance with grocery shopping and meal preparation, our private care team of registered nurses, LPNs, and certified home health aides can provide skilled nursing and assistance with bathing, dressing, preparing meals, or even shopping and other activities.

Hospice Care
Hospice care is a national model of patient- and family-centered, high-quality, compassionate care that helps patients facing a terminal illness and their families live as fully as possible. The VNA of Englewood’s hospice program is certified by Medicare, licensed by the State of New Jersey and accredited by CHAP.
VNA of Englewood utilizes a team approach to deliver comprehensive care that includes skilled nursing, medical social work, personal care, and spiritual guidance. A hospice medical director (medical doctor) is available for consultation 24/7 as well as a nurse practitioner who support the nursing team and patient/family through consults and visits in the home as needed. Hospice care is provided to the patient in their own home, a nursing home, group home or assisted living facility.
Optional and complementary music therapy is available for patients receiving hospice care, which has been shown to assist in alleviating pain, anxiety and fatigue and promoting relaxation and elevated moods. Trained volunteers are also an integral part of the hospice care team, providing support for patients and families and respite for caregivers.

Hospice care utilizes an interdisciplinary team of healthcare professionals and trained volunteers that address symptom control, pain management, and emotional and spiritual support specifically tailored to the patient’s needs and wishes. Palliative advanced illness care
VNA of Englewood’s interdisciplinary team skilled in advanced illness care can help individuals living with chronic illness such as COPD, diabetes, congestive heart failure (CHF), dementia, Alzheimer’s disease and ALS remain healthy at home and avoid unnecessary hospital visits. Services can include skilled nursing care, home-based primary care from a visiting physician, palliative care, rehabilitation therapy, symptom management and education, caregiver respite services and more.
Ruth Hyman Jewish Hospice
The Ruth Hyman Hospice program is conducted under the supervision of a rabbinical spiritual care counselor, who helps provide spiritual and emotional guidance during the hospice journey. Combined with the VNA’s skilled interdisciplinary team, hospice services focus on helping individuals attain the highest quality of life when facing a terminal illness while observing Jewish laws, tradition and values including those who are not affiliated with a congregation. The program is accredited by the National Institute for Jewish Hospice.
Care Choices Program
Medicare beneficiaries with advanced cancers, chronic obstructive pulmonary disease (COPD) or congestive heart failure may be eligible to participate in a new option of hospice care while they continue to receive curative treatments. This program is designed to provide supportive services from the hospice team, assist with patient-family centered goal planning and improve quality of life and patient/family satisfaction. Supporting families through their grief
Our Hearts of Remembrance bereavement support services are available to all family members of patients for up to 13-months. Through certified counselors, chaplains, and volunteers, family members get support to manage grief and work through the bereavement process to honor their loved one’s memory.
