Transitions of Care

Throughout the Transitions of Care experience, PGY1 residents participate in discharge calls with our Population Health Care Coordinators for patients discharged from the inpatient resident service. Additionally, residents participate in home visits with one of the Visiting Nurse Association of Englewood nurses. Residents learn about evidence-based models of care transition, quality discharge planning/counseling, as well as interdisciplinary teamwork and communication through various self-directed learning modules and noon conference series.
